PARENTS OR LEGAL GUARDIANS (AND PARTICIPANT) MUST READ AND SIGN THE FOLLOWING RELEASE IN ORDER TO ATTEND THE YGB FOOTBALL CAMP
I hereby register my child for the YGB Football Camp and authorize the staff to direct him in all camp activities. In consideration of YGB Football Camp by registering my child (or ward) to participate in its football camp, my son has no medical or emotional problems which may affect his ability to safely participate in your program. In the event of injury, I authorize the YGB Football Camp staff and its athletic training staff to obtain and/or administer any medical care or treatment deemed necessary. Neither I, nor my son, will hold the YGB Football Camp and/or Staff members and Jackson Academy, liable for any injuries sustained at the camp.
I also give permission to YGB Football Camp to transport my son (or daughter) from Jackson Academy to Raider Park via school bus to get to and from camp sites throughout the hours of camp. The buses will be operated by a Jackson Academy employee. The distance from Jackson Academy to Raider Park is 0.02 miles on Sheffield Dr.
I give my permission to utilize any camp video or photos that may include my child for any type of advertisement to help better market YGB Football Camp. By signing this, I verify that I am the legal parent or guardian and that I have read and accepted all administrative policies and refund conditions as set forth by the YGB Football Camp that are stipulated on the website and/or in the brochure.
Due to Covid - 19, The Jackson Academy campus already has protocols in place and we will be mirroring or perhaps exceeding their policies on masks, hand - washing, regular monitoring by medical staff and social - distancing as best we can at a football camp.
